Two Cases of Pulsating Exophthalmos


Abstract
Two cases of pulsating exophthalmos are preseted, the one being of idiopathic and the other of post-traumatic origin.
The former, 33 year old man, had been complaining of headache and tinnitus on the Ieft ear fo about five months¢¥ duration, and slight protrusion of the left eye for 3 months. It had been gradually getting worse with the complication of abduscent nerve paralysis and mild glaucoma. The exophthalmometric and tonometric examination revealed 19 mm ((Ls.), 13 mm (0 D.) and 40.5 mmHg (O.S.), 21.5 mmHg (O.D.). The tinnitus disappeared by digital compression of the left carotid artery. The complete roentgenological studies and treatment was refused by the paient.
The latter was the case observed three months after head trauma in 18 year old man.
Immediately following trauma on tile, right head he was admitted to our hospital under the semicomatous state. On the third day of admission, he complained of headache and double vision due to paralysis of the abduscent nerve: Twelve days¢¥ hospitalization resulted gradual¢¥ improvement of general condition, he was discharged still with ;persistent mild headache and double vision.
He began to complain of tinnitus of the right ear on two months after trauma and on three months the progressive exophthalmos of the right eye appeared, so he was re-admitted.
The exophthalmometric and tonometric examination revealed 12 mm (O.S.),17mm (O.D.), 19.5 mmHg (Both eye). Digital compression of the right carotid artery abolished¢¥tinnitus of the right ear and right. Carotid angiogram showed a typical carotid cavernous sinus fistula.
A good result was obtaine by ligation of the right common carotid artery following the application of Matas test for three days.
